Participating Insurance Plans

We participate with most local and many national insurance plans. However it is your responsibility to understand whether your insurance has limits on the doctors you can see, or the services you can receive.

If you provide complete and accurate information about your insurance, we will submit claims to your insurance carrier and receive payments for services. Depending on your insurance coverage, you may be responsible for co-payments, co-insurance, or other deductible amounts. Billing Questions

Thank you for choosing us as your healthcare provider. We are committed to your successful treatment. Please understand that payment of your bill is considered part of your treatment plan. Please call our office and talk with our billing coordinators Ms. Crystal Tucker/ Ms. Sneha Patel or e mail our billing company, at viswanath@ifedora.com with questions or concerns regarding your bill. If unable to reach Ms.Crystal Tucker/ Ms. Sneha Patel, and in case of an emergency, please call our billing company directly at 661-456-3117 or 803-900-9460.

Your Co-Payment is due before you see the doctor. If you have any remaining or unpaid balances these must be paid in full before you see the doctor. While most insurance companies do not require referrals for our services, the HMO plans may need one and so we recommend you contact your insurance company directly to confirm the specifics of your plan. For your convenience we accept cash, checks and most major credit cards. You may even call our office and we will gladly take your payment over the phone.
